Job Title: Home Health Physical Therapist (Spanish-Speaking)

We are currently seeking a skilled and dedicated Home Health Physical Therapist to join our dynamic team. Spanish-speaking ability is required to help us better serve our diverse patient community.
Position Overview:
As a Home Health Physical Therapist, you will play a crucial role in providing specialized rehabilitation services to our patients within the familiar and supportive environment of their homes. You will work closely with patients to assess their physical condition, develop personalized treatment plans, and implement therapeutic exercises to promote recovery, improve mobility, and enhance overall well-being.
This is a PRN position, meaning you will be working with us on an as-needed basis.
You will be hired as a W2 employee, not as an independent contractor.
We offer flexible scheduling, and you can freely choose the areas you prefer to cover.

Key Responsibilities:
  • Conduct thorough assessments of patients' physical conditions to determine rehabilitation needs.
  • Develop and implement individualized treatment plans in collaboration with the healthcare team.
  • Provide hands-on physical therapy interventions to improve patients' strength, flexibility, balance, and coordination.
  • Educate patients and their families on therapeutic exercises, mobility aids, and strategies to promote independent living.
  • Monitor and document patients' progress, adjusting treatment plans as necessary.
  • Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive patient care.
  • Maintain accurate and up-to-date patient records.
  • Communicate effectively with patients, families, and caregivers to ensure a positive and supportive therapeutic environment, with Spanish-speaking skills required to support our Spanish-speaking patients.

Qualifications:
  • Bachelor's or Master's degree in Physical Therapy from an accredited institution.
  • Current state licensure as a Physical Therapist.
  • Prior experience in home health is preferred but not required.
  • Strong clinical and critical thinking skills.
  • Excellent communication and interpersonal abilities, with Spanish-speaking skills required.
  • Ability to work independently and as part of a collaborative healthcare team.
