Home Based Primary Care Physician Role Overview:

The Home-Based Primary Care Physician provides comprehensive primary care services to veterans in their homes, ensuring continuity of care within a multidisciplinary healthcare team. Reporting to the Chief of Primary Care, the provider is a key clinical member within the medical center.

The provider will be responsible for the assessment and care of new and returning patients in the Home-Based Primary Care program. Responsibilities include conducting home visits, developing care plans, coordinating with multidisciplinary teams, and providing follow-up care to ensure optimal health outcomes.

Responsibilities Include:

Veteran-Centered Care:

  • Deliver comprehensive home-based primary care services with an emphasis on health maintenance, disease prevention, patient education, and management of chronic illnesses.
  • Promote shared decision-making with veterans and their families to achieve optimal health outcomes.

Clinical Excellence:

  • Conduct home visits to assess and manage new and returning patients in the Home Based Primary Care program.
  • Develop and implement individualized care plans.
  • Coordinate care with multidisciplinary teams, including nurses, social workers, and other healthcare providers.

Quality Improvement:

  • Participate in continuous quality improvement projects to enhance the delivery of care.
