Key Responsibilities
  • Conduct thorough clinical assessments of patients and their families, developing personalized care plans that address individual needs and preferences while ensuring compliance with hospice care standards.
  • Implement therapeutic interventions using evidence-based modalities, including cognitive-behavioral therapy and solution-focused techniques, to provide emotional support and coping strategies during end-of-life transitions.
