The Licensed Master Social Worker will operate in a remote work setting, allowing flexibility while maintaining consistent support for our clients. Working from home, the Social Worker will engage with clients through virtual platforms, ensuring ongoing communication and seamless care coordination with families and interdisciplinary teams. While regular hours are set for Monday to Friday from 8:00 AM to 4:30 PM, some flexibility in scheduling may be needed to accommodate client visits and urgent situations.

• Conduct thorough assessments of clients' psychosocial needs, focusing on emotional, mental, and social factors that influence their well-being.
• Collaborate effectively with clients and their family members to develop personalized care plans tailored to their unique goals and preferences.
• Provide counseling and support to assist clients in navigating transitions in their lives, addressing challenges such as health issues, loss, or changes in living arrangements.
• Advocate for clients' rights, ensuring equitable access to community resources, healthcare services, and support systems.
• Coordinate with healthcare professionals, caregivers, and support staff to deliver comprehensive care, regularly monitoring clients' conditions and safety.
• Facilitate communication among family members regarding care decisions, addressing conflicts and fostering a collaborative environment.
• Develop and conduct outreach programs that educate clients and families about available community resources, support groups, and social engagement opportunities.
• Maintain meticulous records, ensuring all client assessments, care plans, and interactions are documented in accordance with ethical standards and organizational policies.
• Stay updated on current social work best practices and participate in ongoing professional development activities.

This role primarily serves seniors dealing with various psychosocial challenges associated with aging. The Licensed Master Social Worker will focus on enhancing their emotional well-being and quality of life by addressing issues like isolation, mental health concerns, and access to necessary resources.

• Master's Degree in Social Work from an accredited institution.
• Valid and current licensing as a Licensed Master Social Worker (LMSW) in Pennsylvania.
• At least two years of relevant experience in social work, preferably within a community organization or geriatric setting.
• In-depth knowledge of social work principles, especially as they relate to aging and available community resources.
• Exceptional assessment and problem-solving skills, with a keen ability to empathize and communicate effectively with seniors and their families.
• Proficiency in electronic health record (EHR) systems and other digital tools necessary for client management.

• Experience in hospice care or working directly with dying patients and their families.
• Knowledge of local resources and programs that benefit seniors in the Philadelphia area.
• Familiarity with interdisciplinary teamwork in healthcare settings, particularly regarding communication techniques and collaborative care approaches.
