This position offers a remote work setting that allows you the flexibility to conduct assessments and provide counseling from the comfort of your home while occasionally collaborating with healthcare professionals on-site as needed. Regular business hours will be followed, with occasional evening and weekend shifts to accommodate patient needs and individual caseloads. As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting comprehensive psychosocial assessments through targeted interviews, careful observations, and detailed reviews of medical records to identify the emotional, social, and environmental factors affecting patients' recovery.
• Providing individual and family counseling services aimed at addressing issues related to adjustment, grief, loss, and emotional well-being.
• Collaborating closely with the rehabilitation team—comprising physical therapists, occupational therapists, and speech-language pathologists—to design and implement individualized treatment plans that integrate patients' psychosocial needs with their physical rehabilitation goals.
• Assisting patients in navigating community resources such as support groups, vocational rehabilitation programs, and housing assistance to foster a smooth transition from clinical care to everyday life.
• Participating actively in discharge planning, ensuring that patients receive the necessary support and services upon leaving the facility—be it transitioning to a home environment or another care setting.

Your role will involve working with a diverse patient population, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. These patients often face emotional and social challenges as they work towards regaining independence. Your empathetic approach and specialized social work strategies will be essential in creating a supportive atmosphere that fosters resilience and community reintegration.

The ideal candidate for this role will have:

• A Master's degree in Social Work from an accredited institution, equipping you with the foundational knowledge and skills necessary for the profession.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) in Kansas to practice in this dynamic healthcare setting.

Although not mandatory, the following experiences are preferred:

• Demonstrated experience in rehabilitation or medical settings, which can enhance your ability to address complex patient needs.
• Strong understanding of adjustment issues, grief counseling, and coping mechanisms, including strategies for working with families in crisis.
