JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
Prototypes is one of the nation's leading behavioral healthcare providers and is a "prototype" for such organizations. Prototypes Women's Center in Pomona is a 180-bed residential facility tailored specifically to women, including those who wish to involve their children in their journey to recovery. Our program aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide range of supportive services. Children can foster a secure attachment with their mothers in a secure setting, while also accessing mental health support and educational assistance. Our interdisciplinary team of professionals provides comprehensive treatment for substance use disorders and mental health, parenting, educational, and life skills courses. Treatment can range from 60 days to 6 months.
The Lead Counselor leads staff and collaborates with other members of multidisciplinary team to ensure the best treatment outcome for the clients and provides treatment in accordance with HealthRIGHT 360's philosophy, goals, policies, mission, and vision. The Lead Counselor takes the lead in providing crisis intervention and client stabilization. Other responsibilities may include maintaining facility oversight and safety of clients, monitoring of medications, documenting client activities, performing client drug tests as required, and performing other duties as assigned.
KEY RESPONSIBILITIES
Program Coordination
  • Lead by example and set the tone of responsibility, trauma-informed care, and client customer service.
  • Ensure shift exchange occurs and documented through email for all shift exchanges.
  • Coordinate with other leads and/or supervisors to arrange work schedules around lunch breaks, facility rounds, donations, visiting coverage, transportation, detox round, floor phone assignments, meal monitoring, behavioral rounds, and other responsibilities.
  • Take ownership of any crisis and provide crisis intervention and client stabilization.
  • Oversee the facility and safety of clients.

Direct Service
  • Provide individual or group counseling services and care coordination/case management.
  • Utilize trauma-informed care while applying crisis intervention, motivational interviewing, and supervision of the clients.
  • Assist clients with intake, assessment, and orientation to program admission.
  • Complete facility rounds on a regular basis to maintain supervision of the clients and facility.
  • Complete detox rounds and behavioral support rounds at 30-minute intervals or as assigned.
  • Conduct random and planned room searches.
  • Be available to assist clients who have personal problems or sleeplessness.
  • Drug test clients as required.
  • Assists with community outings.
  • Monitor medication line as needed or as assigned.
  • Provide facility coverage by supervising clients during chores, visiting hours, mealtimes, medication line, detox, donations, groups, and free time.
  • Monitor visiting hours and address any safety concerns.
  • Transport clients as needed.

Clinical Documentation
  • Write and complete all progress notes within 72 hours of service delivery.
  • Maintain compliance with the due dates for all clinical documentation.
  • Documentation must be maintained in compliance with agency policy and procedures, HIPAA, 42-CFR, DMH, and SAPC standards to satisfy internal and external evaluating requirements.
  • Document all client UA's using the UA form and a progress note.
  • Complete same-day incident reports.
  • Document detox rounds and behavioral support rounds every 30 minutes as assigned.
  • Review documentation of facility rounds to ensure all required rounds during the shift occurred.

Communication and Collaboration
  • Seek clinical consultation when encountering new and/or high-risk clinical circumstances.
  • Work and communicate collaboratively with other members of the treatment team and communicate any behavioral concerns to the client's Case Manager, Clinical Therapist, Medical, and Psychiatrist.
  • Maintain frequent communication with the treatment team and engage in regular consultations.
  • Immediately inform the manager on-call of a situation that is necessary and/or could need medical assistance.

Administration and Other Duties
  • Check all areas in the chart room for any filing needed.
  • Attend and actively participate in assigned supervision.
  • Maintain full compliance with requirements for registration or certification.
  • Actively participate in agency and team meetings.
  • Pursue training in evidence-based practices.
  • Complete all assigned training in a timely manner.
  • Read and respond to emails when appropriate.
  • Arrange work schedule in accordance with the agency's needs.

And perform other duties as assigned.
QUALIFICATIONS
Education, Certification, or Licensure
  • High school diploma or equivalent required. Bachelor's degree in Health and Human Services, Social Work, Psychology, or other related field preferred.
  • Registration or Certification as a Substance Use Disorder Counselor from an accredited California agency (CCAPP, CAADE, CADTP).
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Valid California Driver's License and access to registered and insured transportation.

Experience
  • Minimum of two years' experience as a counselor in substance abuse treatment, relapse prevention and recovery.
  • Experience providing individual and group counseling sessions.
  • Experience as a supervisor or lead for at least one year preferred.
  • Experience delivering evidence-based practices.
