Job Title: RN-Homecare/Hospice
Location: West Point, NE
Duration: 13 weeks
Schedule: Variable - Days

Travel Gross weekly: $2250 – 2350/week (Based on Exp)

Local Pay: $57-60/hr (Based on Exp)

Job Description:

RN needed ASAP for homecare/hospice 30 mile radius from West Point, NE. Will visit 3-5 homes per day. Vehicles and gas are provided to our travelers by the facility Mon- Fri with every 4th weekend on callResponsibilities
  • Provide skilled nursing care to patients in their homes.
  • Provide hospice and end-of-life care as appropriate.
  • Assess patient conditions and develop/implement individualized care plans.
  • Administer medications and treatments as ordered.
  • Monitor patient progress and communicate changes to the healthcare team.
  • Educate patients and families regarding medications, treatment plans, and disease management.
  • Maintain accurate and timely clinical documentation.
  • Coordinate care with physicians, therapists, aides, and other healthcare professionals.
  • Respond appropriately to patient needs during scheduled on-call periods.
  • Follow all applicable nursing, home health, hospice, and facility policies.
Requirements
  • Active RN license eligible to practice in Nebraska.
  • Previous home health and/or hospice nursing experience preferred/required.
  • Strong clinical assessment and documentation skills.
  • Ability to work independently and manage multiple home visits.
  • Excellent communication and interpersonal skills.
  • Valid driver's license and ability to travel to patient homes.
  • Ability to work Monday–Friday and participate in every 4th weekend on-call rotation.
  • Must be available to start ASAP.
