While the summer months are often considered slower in Phoenix, our Skilled Nursing Facility and outpatient clinic continue to experience strong demand - we are fully booked with an active patient waitlist. Enjoy flexible scheduling; achieve work-life balance on your terms, working either one or two days per weekend. Consistent hours available! This weekend PRN Physical Therapist will work inpatient Med A evals for our skilled nursing facility.

Join an amazing in-house team dedicated to enriching the lives of seniors through compassionate care and innovative programs. As a Physical Therapist, you'll provide exceptional services within Assisted Living, Skilled Nursing, and Memory Care residents. This role will foster a positive, supportive environment for residents, families, and team members on a PRN basis.

Pay Rate: $42 - $65 per hour

What you'll do:

โ€ข Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.

โ€ข Responsible for the correct reporting of billing information of Physical Therapy in regard to all patients on personal case load.

โ€ข Provide direct patient care as prescribed, including evaluation and treatment planning.

โ€ข Responsible for the reporting and interpretation of treatment results.

โ€ข Participate in development and presentation of in-services for the facility as requested.

โ€ข Supervise support personnel in immediate work areas as appropriate.

โ€ข Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Facility's policies and procedures.

โ€ข Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum.

โ€ข Manual manipulation, including pushing and/or pulling of patient body parts and limbs.

โ€ข Participate in meetings regarding patient care as appropriate.

โ€ข Assist when appropriate with any community education and public relations activities of the facility.

โ€ข Maintains a high standard of resident care.

โ€ข Responds immediately to all crisis and/or calls for assistance.

โ€ข Actively participates in in-services, department meetings and continuing education.

โ€ข Provide education to patient's family as well as staff on the specific treatment plan as well as therapy goals for the patient.

โ€ข Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.

A minimum of Doctorate of Physical Therapy. Sedentary tasks related to job โ€“ approximately 10% Physical tasks as listed โ€“ approximately 90%. CPR certified. Licensed by the State in the designated geographic area in their respective therapy profession.
