As a Weekend Hospice RN, you will coordinate patient assessments, manage symptoms, and support families with empathy and professionalism. Duties

Conduct comprehensive patient assessments, including vital signs, symptom management, and psychosocial evaluations.

Administer medications and treatments such as IV infusions, pain management protocols, and wound care, ensuring adherence to hospice standards.

Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with patient wishes and clinical needs.

Provide education and emotional support to patients and their families, guiding them through complex medical decisions and end-of-life processes.

Maintain accurate medical documentation within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.

Respond promptly to urgent patient needs via triage protocols, coordinating with emergency services when necessary.

Participate in ongoing training related to hospice & palliative medicine, infectious disease care, and advanced symptom management techniques such as opioid titration or ostomy care.

Requirements

Valid RN license in the state of practice with
