RN - Admissions Case Coordinator Position Summary
The Admission Case Manager is an active member of the Patient Care Management and ED team. The role combines every function of case management, from Access through Discharge planning, and post-discharge from the ED. The case manager serves, manages, and supports internal and external customers, communicates appropriately using good interpersonal skills, participates in performance improvement initiatives and programs, participates as a team member, and is accountable for own work responsibilities.
Three core responsibilities of the Admission Case Manager are to provide:
  • Consultation and direction for physicians about the appropriate determination of the level of care, specifically Observation vs. Inpatient status.
  • Discharge planning directly from the ED to levels such as Acute rehab, SNF, and homecare services.
  • Development and implementation of care plans for those patients who visit the ED frequently.

RN - Admissions Case Coordinator Position Requirements
Education
  • Bachelor's degree preferred.

Required License and/or Certification
  • Current RN license in the state in which you reside.

Experience
  • Minimum of 1 to 3 years experience in an acute care hospital.
  • Home health, case management experience with an insurance carrier or HMO and/or Utilization Review and discharge planning experience preferred.

RN - Admissions Case Coordinator Schedule
  • 12 hour shifts with 24/7 coverage.
  • Assigned shift may require rotation to opposite shift.

RN - Admissions Case Coordinator Benefits
  • At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
