Schedule:
Monday–Friday | 9:00 AM – 5:00 PM
No nights or weekends
This role is open exclusively to New York State–licensed Registered Nurses (RNs).
Job Overview:
We are seeking a compassionate and clinically strong Registered Nurse (RN) to provide community-based care for adults living with serious mental illness in Ulster County, NY. This role is part of a collaborative mobile team that delivers outreach, nursing support, care coordination, and clinical follow-up in the community.
This is an excellent opportunity for an RN looking for meaningful work, a stable weekday schedule, and a team-oriented environment outside of a traditional hospital setting.
RN Responsibilities:
  • Provide outreach and nursing support in a community-based care model
  • Conduct mental and physical health assessments
  • Administer and monitor medications in collaboration with a Psychiatrist
  • Coordinate referrals and follow-up care based on individual needs
  • Participate in team meetings and interdisciplinary care planning
  • Maintain timely, accurate clinical documentation
