Position Overview

Vision Works Youth Counseling Center is seeking a compassionate and skilled Licensed Independent Clinical Social Worker (LICSW) to join our dedicated team in Tacoma, Washington. This full-time remote position offers an exciting opportunity for a social worker who is committed to providing high-quality mental health support in a hospice setting. The successful candidate will engage with our clients to assess their psychosocial needs and develop tailored interventions aimed at improving their emotional well-being.

Work Environment

This remote position allows for flexibility in work hours while ensuring that our clients receive the care and attention they need. Your work schedule will primarily be Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to respond to client requirements. Part-time options may be negotiable based on the needs of clients. Key Responsibilities
  • Conduct thorough assessments of the psychosocial needs of senior clients, considering emotional, mental, and social factors.
  • Develop individualized care plans in collaboration with the clients and their families, focusing on their preferences and specific goals.
  • Provide counseling and emotional support to clients and their families to help them navigate transitions, challenges, and crises.
  • Advocate for clients’ rights and assist them in accessing relevant community resources to ensure they receive comprehensive care.
  • Work as part of an interdisciplinary team, collaborating closely with nurses, caregivers, and healthcare professionals to enhance service delivery and overall client care.
  • Conduct regular home visits to monitor clients’ well-being and living conditions, addressing concerns and safety issues promptly.
  • Facilitate open communication and mediate conflicts among family members involved in the care of senior clients.
  • Educate clients and families about available community resources, support groups, and programs that can enhance their social engagement.
  • Maintain detailed documentation of client assessments, care plans, progress notes, and outcomes while adhering to regulatory and ethical standards.
  • Stay informed about current best practices, policies, and regulatory requirements in social work, ensuring compliance at all times.
