• Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, creating individualized service plans tailored to their needs.
• Engage in individual and family counseling sessions, providing support and guidance for clients experiencing various life challenges, such as mental health crises or social instability.
• Develop client-centered goals and interventions, frequently monitoring and adjusting service plans based on progress and changing needs.
• Provide crisis intervention and manage risk by developing safety plans during acute emotional distress.
• Collaborate with community resources, agencies, and organizations to ensure clients have access to a wide range of services, including housing, healthcare, and social support.
• Involve in community outreach and engagement initiatives, fostering awareness of available agency resources and contributing to preventative measures targeting social issues.
• Regularly participate in case conferences, team meetings, and staff training to enhance professional skills and contribute to a collaborative work environment.

In this role, you will serve a diverse population of individuals and families facing significant life challenges, including those struggling with mental health issues, substance use disorders, and socioeconomic hardships. • A Master’s degree in Social Work from an accredited program, ensuring a strong foundation in social work principles and practices.
