• Conduct comprehensive assessments to evaluate clients' social, emotional, and psychological needs, ensuring personalized service plans that reflect their unique circumstances.
• Provide one-on-one counseling and group therapy sessions utilizing evidence-based therapeutic approaches tailored to clients facing end-of-life challenges.
• Develop, implement, and monitor service plans with clients and families that are responsive to their needs and aligned with established goals.
• Advocate proactively for clients’ rights and access to resources, addressing social justice issues as they arise.
• Collaborate effectively with a multidisciplinary team, including medical professionals and community outreach partners, to provide holistic care and enhance service delivery.
• Document interactions and outcomes meticulously for compliance with program evaluations and grant reporting.
• Engage in ongoing professional development and training opportunities to remain current in best practices and emerging trends in social work and hospice care.

This role focuses on providing empathetic support to patients in hospice care, as well as their families. These clients often face emotional distress related to terminal illness, grief, and family dynamics. Your role will help clients navigate these complex feelings while promoting dignity and comfort during this challenging time.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• A thorough understanding of social justice, trauma-informed care, and advocacy in a healthcare setting.
• Strong interpersonal skills, empathy, and the capacity to communicate effectively with diverse populations.
• Excellent organizational and documentation skills are essential for maintaining detailed records and service plans.

• Previous experience in a nonprofit organization or healthcare-focused setting is highly preferred.
• Familiarity with hospice care and end-of-life issues will be an asset.
• Experience developing program initiatives and collaborating with community resources will be advantageous.
