Key Responsibilities
  • Conduct comprehensive assessments to evaluate clients' social, emotional, and psychological needs, ensuring personalized service plans that reflect their unique circumstances.

  • Develop, implement, and monitor service plans with clients and families that are responsive to their needs and aligned with established goals.

Patient Population / Client Focus

This role focuses on providing empathetic support to patients in hospice care, as well as their families. Your role will help clients navigate these complex feelings while promoting dignity and comfort during this challenging time.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • An active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A thorough understanding of social justice, trauma-informed care, and advocacy in a healthcare setting.
  • Strong interpersonal skills, empathy, and the capacity to communicate effectively with diverse populations.
