Our nurses provide physician-ordered care while building meaningful relationships with patients and families. Through clinical excellence and personalized support, we help reduce hospitalizations and improve quality of life.

RN/LPN Essential Responsibilities

  • Provide skilled in-home nursing care for low- to high-acuity patients

  • Perform comprehensive patient assessments and follow individualized plans of care

  • Administer medications and treatments as prescribed

  • Collaborate with families and interdisciplinary team members to support positive patient outcomes

  • Accurately document care and communicate patient changes to the RN Clinical Supervisor/Site Director

  • Safely lift, position, and transfer patients, supplies, and equipment

Qualifications

  • Active Arizona RN or LPN license, or Compact License

  • Valid Arizona Class D Driver's License

  • Current BLS Certification (online-only courses not accepted)

  • Current TB test or chest X-ray within the past 12 months

  • Level One Fingerprint Clearance Card

  • Article 9 Training (assistance available)

  • Strong time management, communication, and problem-solving skills
