Job Description
Title: Case Manager RN - Acute Hospital
Schedule:
- Shift: 5x8 Days, with shift times between 07:00-17:00
- Weekends: Yes, required every other weekend
Assignment Details:
- Guaranteed Hours: 72 hours per 2 weeks
Requirements:
- License: Current RN license in the state required
- Experience: Minimum of 1 year acute care hospital experience required
- EMR: Epic experience required
- Certifications: ACM certification preferred; BLS preferred
- Skills: Discharge planning, care coordination, utilization review; familiarity with InterQual and Milliman criteria; basic ICD-10 familiarity
Roles & Responsibilities:
- Coordinate discharge planning and care for diverse patient populations, including neonates, pediatrics, adults, and geriatrics.
- Collaborate with interdisciplinary teams including physical therapy, pharmacy, social services, and others involved in patient care.
- Conduct utilization review and continued stay reviews to ensure appropriate patient care and resource management.
- Maintain thorough documentation within the electronic medical record system.
Compliance / Notes:
- AK RN license in hand required for Auto Offer; pending license may be acceptable for submission
- Time-off request: Only 1 time-off request per 13-week assignment; no holiday RTO for Auto Offer
- Holiday coverage: Willingness to work 2 of these 3 holidays - Thanksgiving, Christmas, New Year
- Assignment limits: Max 1 calendar year at facility, then 6-month break before returning
