Position Overview

As a Mobile Medical Nurse Practitioner, you will provide primary care services to patients in their homes throughout Hernando County, FL.


This role is ideal for experienced Nurse Practitioners who enjoy working independently, managing chronic conditions, and developing long-term relationships with patients in a community-based setting.

Primary Responsibilities

  • Perform comprehensive in-home health assessments
  • Diagnose and treat acute and chronic medical conditions
  • Develop individualized treatment and care plans
  • Order, review, and interpret laboratory and diagnostic testing
  • Prescribe medications and monitor treatment effectiveness
  • Educate patients and caregivers regarding disease management and preventive care
  • Coordinate care with physicians, specialists, home health agencies, and interdisciplinary teams
  • Maintain accurate and timely electronic medical records documentation
  • Support preventative care and population health initiatives

Qualifications

Required

  • Master's Degree from an accredited Nurse Practitioner program
  • Active Florida APRN License
  • National Board Certification as:
    • Family Nurse Practitioner (FNP)
    • Adult-Gerontology Nurse Practitioner (AGNP)
    • Adult Nurse Practitioner (ANP)
  • Current DEA Registration (or ability to obtain)
  • Current Professional Liability Insurance
  • Valid Florida Driver's License and reliable transportation
