The Nurse Practitioner supports patients and families during the final stages of illness, offering thoughtful medical care and a steady, compassionate presence. By managing medications, treatments, and recertifications, Nurse Practitioners provide continuity and stability, allowing families to focus on comfort, dignity, and meaningful time together.

Nurse Practitioner Requirements & Key Responsibilities:
  • Must have current Nurse Practitioner (NP) license in the state(s) of practice.
  • Degree from an accredited NP program required.
  • Minimum two years of RN experience; preferred hospice or palliative care experience.
  • Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
  • Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.
  • Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
  • Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
  • Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
  • Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
  • Valid driver's license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
