Job Description

The Nurse Practitioner performs health assessments and preventative health measures within prescribed guidelines and physician instructions. They will order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs. They will further discuss the cases with the physician and formulate and document care plans. Further responsibilities include prescribing medication or other forms of treatment.

Additional Details:

  • Medicare population with focus on Chronic Diseases
  • 12-15 patients per day  
  • Will be responsible for full assessments of Chronic Health patients (COPD, Diabetes, HTN)
  • Monitor and treat CHF and uncontrolled HTN patient population
  • EMR system is NexGen

Hours for this Position:

Monday - Friday 8:00am - 5:00pm, on-call one weekend a month

Qualifications
  • Master's Degree in Nursing 
  • Unrestricted RN license and NP license in AZ
  • 1 + year of NP experience with clinical emphasis 
  • National Board Certification within 1 year and if applicable in your governing state
  • 2-5 years of experience in acute care settings such as: ICU, CCU, telemetry,or med-surgical
  • Must have prescribing authority/DEA license in good standing
