Licensed Practical Nurse (LPN)

Health & Social Services | Yuuuiath Government

Classification: Band 5 - Specialist / Senior Technical

Reports To: Community Care Nurse (RN)

Location: Hitacu, British Columbia and within Yuuuiath community settings

Employment Type: Full-time

Salary Band: Band 5 ($61,800 - $92,700 $31.69 - $47.54/hr)

POSITION PURPOSE

The Licensed Practical Nurse (LPN) provides direct nursing care to community members across the lifespan, working within their full scope of practice under the direction of the Community Health Nurse. The LPN supports clinical assessments, medication administration, wound care, chronic disease management, and health promotion in homes, the health centre, and community settings. The role is grounded in cultural safety and trauma-informed care, supporting the wellness goals of the Yuuuiath Government.


ACCOUNTABILITY

Clinical Nursing Care & Assessment

  • Provide direct nursing care including medication administration, wound care, and injections within LPN scope.
  • Monitor vital signs and support chronic disease management to BCCNM standards.
  • Conduct focused health assessments and document findings.
  • Report changes in client condition to the Community Care Nurse and escalate emerging concerns.

Care Planning & Team Collaboration

  • Contribute to individualized care plans developed with the Community Care Nurse.
  • Implement assigned components of care plans across home, clinic, and community settings.
  • Work collaboratively with the Nurse, HCAs, Homemakers, and external health partners.
  • Participate in team meetings, case conferences, and care planning discussions.

Health Promotion & Education

  • Support immunization clinics, wellness events, and chronic disease education.
  • Provide culturally appropriate health information to clients and families.
  • Reinforce self-management strategies for clients with chronic conditions.
  • Ensure continuity of care through clear handovers and team communication.

Documentation, Compliance & Cultural Safety

  • Maintain accurate, timely, and confidential client records.
  • Comply with BCCNM, FNHA, and YG documentation policies.
  • Deliver care that is culturally safe, trauma-informed, and respectful of Yuuuiath traditions, language, and family structures.
  • Build trusting relationships with clients, families, and community members.


EDUCATION & EXPERIENCE

Required:

  • Active practicing registration as a Licensed Practical Nurse with the BC College of Nurses and Midwives (BCCNM)
  • Graduation from a recognized Practical Nursing program
  • Current CPR Level C certification
  • Valid Class 5 Driver's License and access to reliable transportation
  • Criminal Record Check (vulnerable sector) clearance

Preferred:

  • Previous experience in community health, home care, or primary care nursing
  • Experience working with Indigenous communities and knowledge of First Nations health frameworks
  • Foot care, immunization, or chronic disease management certifications
  • Experience with electronic medical records (EMR) systems
