Our Nurse Practitioner (NP) providers become an integrated part of each community—partnering with residents, families, nursing staff, therapists, and collaborating physicians to reduce avoidable hospitalizations while improving quality outcomes. Nurse Practitioner (NP) play a vital role in improving patient outcomes and delivering exceptional clinical care.

What You'll Do

As the primary on-site Nurse Practitioner (NP) provider for your assigned facilities, you will:

  • Provide comprehensive primary care to residents in skilled nursing, long-term care, and assisted living settings.

  • Perform initial evaluations, routine follow-up visits, annual wellness visits, and CMS-required skilled nursing facility visits.

  • Assess and treat acute changes in condition with a focus on treating residents safely in place whenever appropriate.

  • Manage chronic diseases, medication reconciliation, and deprescribing initiatives.

  • Collaborate closely with facility nursing teams, therapists, pharmacy consultants, families, and collaborating physicians.

  • Conduct advance care planning and goals-of-care discussions.

  • Participate in interdisciplinary care conferences and quality improvement initiatives.

  • Complete accurate documentation in Hero's electronic health record within required timeframes.

  • Support transitions of care and reduce avoidable hospital readmissions.

  • Participate in Hero's quality and value-based care programs.

Required Qualifications

  • Master's degree or higher from an accredited Nurse Practitioner program

  • National board certification (FNP, AGNP, AGPCNP, or GNP)

  • Active Missouri APRN and RN licenses

  • Eligible to practice under a Missouri Collaborative Practice Arrangement

  • DEA and Missouri BNDD registration (or ability to obtain)

  • Medicare (PECOS) and Medicaid enrollment eligibility

  • Current BLS Certification

  • Valid driver's license, reliable transportation, and current auto insurance

  • Experience using Electronic Health Records (EHR)

Preferred Qualifications

  • Two (2) or more years of Nurse Practitioner (NP) experience

  • Experience in Family Practice, Adult, Skilled Nursing, Long-Term Care, Geriatrics, Home-Based Primary Care, or Acute Care

  • Experience with PointClickCare or other Long-Term Care EHR systems

  • Knowledge of CMS Skilled Nursing Facility regulations

  • Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles

Compensation

  • Competitive Salary Range: $125,000-$150,000

  • Performance Incentive Program through Hero's Provider Incentive Compensation Plan
