Responsible for utilizing the nursing process to provide direct patient care to an assigned group of patients during a shift. Responsible for prioritizing the delivery of direct nursing care using time and resources efficiently integrating a standards-based framework model.

Responsibilities:

  • Acts as clinical resource by overseeing the delivery of patient care, directs and delegates patient care assignments to home health aides and nursing staff based on acuteness of patient care needs and staff capability and anticipates future needs
  • Maintains clinically competent in providing direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care..
  • Coordinates patient care through collaboration with the patient, family, all other disciplines, primary and secondary physicians and community resources/programs.
  • Demonstrates knowledge philosophy and provides support to patients and their families.
  • Ensures that patients understand their rights and responsibilities, advanced directives and the meaning of informed consent during the admission process.
  • Assesses and directs patient and family teaching needs and is proficient in the use of return demonstration and teaching aids in the home setting.
  • Maintains professional responsibility for knowledge of current nursing research, practice standards, guidelines and protocols, and hospital policies, procedures, and programs to improve patient care quality.
  • Maintains a proficiency in the use of all equipment used in his/her practice.
  • Effectively communicates and assists other members of nursing team and/or other disciplines. Actively participates in problem solving and conflict resolution to maintain team cohesiveness.
  • Performs on-call duties effectively with the ability to triage patients over the phone and visit when needed.
  • Directs own educational needs for the top 10 ICD9 codes and/or diagnoses for diabetes, wounds/post-surgical, arthritis, newborns, Coumadin dependency, malnutrition/ nutrition needs, medication instruction, pain and symptom control/hospice care.
  • Supervises the home health aides in compliance with the Medicare/Medicaid rules and regulations.
  • Performs other functions as required.

Qualifications:

Academic Preparation:

  • Degree from accredited academic nursing program (If a program is under conditional status, approval may be granted by system CNO)
  • Minimum ADN preparation


Licensure: State of New Mexico or Compact State Nursing License required.

  • Required Certification from either AHA, ASHI, or Red Cross
  • BLS: All RN roles
  • ACLS: Emergency Department, Critical Care, Cardiovascular Lab, GI Lab, Interventional Radiology, Progressive Care, Surgical Services Pre-Op and Recovery Room
  • NRP: Family Birthing Center, Labor and Delivery, Neonatal Intensive Care, Mother/Baby Care
  • PALS: Children Services, Cardiovascular Lab, Emergency Department, GI Lab, Interventional Radiology, Surgical Services Pre-Op and Recovery Room
