JOB TITLE:  Home Infusion Registered Nurse - West Valley

EMPLOYER: Twelfth One, LLC dba Aspen Infusion

REPORTS TO: Field Nurse Supervisor

SUMMARY: Provides skilled professional nursing services to a case load of patients in their place of residence in accordance with the patient's plan of care. 


DUTIES AND RESPONSIBILITIES:

  • Assesses patients prior to provision of care.
  • Provides ongoing assessment of the patient and patient's equipment needs.
  • Initiates timely discharges.
  • Educates and instructs the patient, patient's family, or staff as required; counsels patient and family in meeting patient's needs and goals.
  • Prepares timely, legible, and complete documentation of all care, interventions, and care coordination as provided by law, regulation, and established policy; returns documentation to the office/department according to established policy. 
  • Demonstrates an awareness of and sensitivity to patient/family rights.
  • Applies safety principles as identified by established policy.
  • Participates in the orientation and training of new staff.
  • Participates in care coordination meetings.
  • Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies/practices.
  • Ensures compliance with legal issues, including but not limited to patient confidentiality and risk management; ensures compliance with ACHC/HIT, federal, state, and local regulations.
  • Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.
  • Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality. 
  • Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.
  • Performs other related duties as assigned by management. 

Requirements

QUALIFICATIONS:

  • Must possess a current Arizona RN license.
  • Must possess a current BLS certification.
  • Minimum of one year related experience or equivalent; infusion nursing experience is preferred.
  • Must possess a valid driver's license and auto insurance.
  • Proof of TB screening.
  • Demonstrated competence in reacting to and handling emergencies.
  • Strong charting/documentation skills.
  • Ability to understand and adhere to established policies, procedures, and protocols.
  • Commitment to excellence and high standards

  • Strong organizational, problem-solving, and analytical skills
  • Ability to manage priorities and workflow
