Job description

OVERVIEW

The Home-Based Caseworker/Specialist provides comprehensive home-based services to children and families throughout Vigo, Lake, and Madison Counties. This role focuses on connecting families to resources, strengthening life skills, and supporting goal achievement to help families sustain independence and improve daily functioning.

Services are individualized, strength-based, and focused on enhancing resilience, improving family dynamics, and addressing barriers related to housing, employment, communication, and overall family stability.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Assesses the needs of children and families to establish short- and long-term goals
  2. Connects families to community resources including housing, employment, and social services
  3. Provides mentorship, advocacy, and support to ensure client success
  4. Assists families in developing life skills, coping skills, and healthy relationships
  5. Supports families with budgeting, parenting education, and problem-solving strategies
  6. Creates and maintains safety and treatment plans in Case Management Pro
  7. Documents all sessions, case notes, and reports within required timelines
  8. Monitors and addresses safety concerns and provides crisis intervention as needed
  9. Documents use of evidence-based practices (e.g., Motivational Interviewing)
  10. Ensures client understanding and compliance with program expectations and handbook
  11. Maintains consistent communication with clients and team members
  12. Participates in team meetings, trainings, and supervision as required
  13. Adheres to all agency policies, confidentiality standards, and DCS requirements

MINIUM REQUIRMENTS:

Education

  • Bachelor’s or Master’s degree in social work, psychology, human services, or related field preferred

OR

  • High school diploma, GED, or Associate’s degree with at least 4 years of direct experience providing casework services to children and families

0-Experience

  • Experience working with children and families preferred
  • Experience in home-based, child welfare, or community-based settings strongly preferred

Skills and Competencies

  • Strong communication (verbal and written)
  • Ability to build rapport and engage families
  • Crisis management and problem-solving skills
  • Ability to work independently in community-based settings
  • Strong organization and time management skills
  • Ability to collaborate effectively with internal and external stakeholders
  • Understanding of trauma-informed and strength-based approaches

Additional Requirements

  • Must be at least 21 years of age
  • Valid driver’s license, reliable transportation, and current auto insurance
  • Ability to pass background checks and all required clearances
  • CPR and First Aid certification (or ability to obtain)
  • Ability to work a flexible schedule, including evenings and weekends
  • Must be trained and competent in required evidence-based practices

SCHEDULE:

This position requires a flexible schedule to meet the needs of families.

  • Services are primarily delivered in the home and community
  • Typical working hours range between 9:00 AM and 7:30 PM based on client availability
  • Evening hours are regularly required
  • Occasional weekend availability may be needed
  • Schedule will vary based on caseload and program demands
