Responsibilities

Works with neonate, adolescent, adult, and geriatric patients, families, medical staff and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess posthospital care needs.  Provide support to address social drivers of health.

Qualifications

REQUIRED

  • Graduate of an accredited Registered Nursing School (RN)
  • Current License in the State of Georgia as a Registered Nurse or Nursing Compact (NLC/eNLC) or Multistate License
  • Two (2) years previous clinical experience.
  • PREFERRED

  • BSN
  • Three (3) years of experience in case management, social services or related field.
  • Certified Case Manager.
  • Ability to interact with patients, families, physicians and hospital staff.
  • Excellent oral and written communication skills.
  • Ability to type accurately at an approximate rate of 30 wpm.
  • Work Hours:8AM-430PM Weekend Requirements:NoOn-Call Requirements:NoEmployment Type: FULL_TIME
