Licensed Practical Nurse (LPN)

Every weekend - Saturday & Sunday

8-10 visits per day

We are are seeking a Licensed Practical Nurse (LPN) to provide high-quality, compassionate in-home patient care. This role supports Registered Nurses and physicians in delivering individualized care plans while ensuring patient comfort, safety, and dignity. The schedule for this role is every weekend, Saturday & Sunday, must reach visit requirements each day.

Key Responsibilities
  • Assist the Registered Nurse (RN) in implementing patient plans of care
  • Perform nursing procedures as directed by the RN and under physician orders
  • Assist with development, implementation, and ongoing evaluation of care plans
  • Monitor, observe, document, and report patient condition, symptoms, and changes
  • Provide emotional support and ensure patient comfort and safety
  • Educate patients and families on appropriate self-care techniques
  • Communicate effectively with interdisciplinary care team members
  • Prepare and maintain accurate clinical and progress documentation (excluding admissions)
  • Follow all company policies, state regulations, and clinical standards
Qualifications
  • Active Licensed Practical Nurse (LPN) license in the state of practice
  • Graduate of an accredited practical nursing program
  • Home care, long-term care, or clinical experience preferred
  • Strong assessment, documentation, and communication skills
  • Ability to work independently in a home care environment
  • Compassionate, patient-focused approach
  • Reliable transportation and ability to travel to patient homes
