Job description

Employment Status: Full-time | Shift: Weekends Only | Location: 1906 Belleview Ave SE - Roanoke

Requisition Number: R160315

Sign on bonus and relocation assistance available.

Three 12‑hour shifts: Thursday, Friday, Saturday, 7am–7pm.

Responsibilities

The RN Case Manager provides comprehensive case management for assigned patient populations, ensuring cost‑effective, coordinated care and timely discharge planning. Key duties include:

  • Performing patient needs assessments at admission and at regular intervals.
  • Initiating referrals and linking patients to health, wellness, and post‑acute resources.
  • Promoting interdisciplinary collaboration and participating in daily team meetings.
  • Managing patient status using InterQual software and supporting accurate documentation.
  • Communicating with patients, families, and external partners to explain financial and discharge implications.
  • Coordinating post‑acute referrals (home health, hospice, DME) and securing prior authorizations.
  • Facilitating transitions to and from outside facilities, including jails or prisons.
  • Contributing to departmental performance‑improvement initiatives and predictive analytics.
  • Maintaining effective communication with Utilization Review nurses, social workers, and other disciplines.
  • Ensuring compliance with federal, state, and CMS discharge regulations.
Qualifications

Education: Registered Nurse. Bachelor’s degree required; 5 years of RN experience may be considered in lieu of a degree.

Experience: Minimum three years of recent clinical experience with direct management of patient care, including education, psychosocial support, discharge planning, and utilization management. Supervisory or leadership experience preferred.

Licensure & Certifications: Current Virginia RN license; AHA BLS‑HCP required within 6 months of hire.

Competitive compensation range: $76,689.60 – $115,044.80.
