Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. As a self-directed team member with Home Living Continuing Care in the Edmonton zone, the Registered Nurse works collaboratively with the health care team and has responsibilities, including assessment of needs, delivering appropriate therapeutic nursing services within professional practice standards, coordinating resources, establishing goals for client delivery, and evaluating outcomes. In addition, this position provides case management as well as direct client care that meets established clinical standards, and is evidence based and client centered. In collaboration with family and other partners, the Registered Nurse acts as a consultant on the team providing clinical expertise and direction to other team members regarding client cases, developing a care plan, based on those unmet needs, using the appropriate internal and external resources. As a member of the multidisciplinary team, the Registered Nurse is responsible for participating in staff development and mentoring student clinical experiences. The Triage/Float RN role ensures timely efficient access to care for community clients & serves as the first point of contact for incoming referrals and inquiries, supporting the home care interdisciplinary team. The successful candidate will perform triage, case management, and direct nursing interventions. This position is based out of the St. Albert Home Care office but will provide support to the Suburban/Rural West team, which includes travel to other home care network offices, as required.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.
