Job description

Coordinated Entry Specialist

Summary

To complete/enter data into HMIS (Homeless Management Information System) and complete Coordinated Entry Assessments to assure a high level of data quality across the system, and to serve as liaison with consumers and homeless service providers.

Position Type

Non-Exempt/Full-time

Essential Functions

· Enter client data in the LSNDC HMIS Data Base

· Work with System Administrator in generating required reports

· Develop expertise in using the Advanced Reporting Tool

· Complete program intakes forms

· Refer consumers to eligible services

· Establish a rapport to engage consumers to services

· Report any significant concerns to Supervisor

· Maintain training requirements

· Develop and maintain a comprehensive housing and resource database.

· Maintain assigned program records.

· Projects and duties as assigned

Supervisory Responsibility

This position has no supervisory responsibilities

Required Education and Experience

Proficiency in a variety of computer applications and systems is required. A high school diploma or equivalent is required, along with a minimum of two years of experience working with individuals experiencing homelessness or other vulnerable populations. The position requires excellent communication and interpersonal skills, as well as the ability to work effectively both independently and under supervision.

Physical Demands

While performing the duties of this job, the employee is frequently required to stand; walk; sit, use hands, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. This position is performed primarily in a shelter environment serving individuals experiencing homelessness. The work involves regular interaction with program participants, staff, and community partners in a dynamic, service-focused setting. The employee may occasionally be exposed to outdoor weather conditions. The noise level in the work environment is usually moderate.
