VNA of Ohio is one of Northeast Ohio's most established home health agencies - 124 years strong, CHAP-accredited, and 4-star rated by CMS. We're looking for a compassionate, skilled Bereavement Coordinator/Chaplain to join our Hospice team in a PRN, field-based capacity serving patients, families, and bereaved loved ones throughout the Cleveland / Northeast Ohio area.


Pay Rates โ€“ Per Completed Visit

Per Visit - $50

Downtime (pre-approved wait time) - $30/hr

"Completed" = visit done in the home or community setting + documentation finished in EMR within 24 hours + required follow-up uploaded + meets QA standards.

What You'll Do
  • Provide emotional, spiritual, and bereavement support to hospice patients and their families before and after the loss of a loved one
  • Conduct bereavement assessments and develop individualized bereavement care plans
  • Deliver in-home and community-based bereavement visits, calls, and follow-up in accordance with the plan of care
  • Facilitate grief support groups or memorial services as needed
  • Collaborate with the interdisciplinary hospice care team to ensure holistic, whole-person care
  • Maintain accurate and timely documentation in the EMR within required timeframes
  • Ensure bereavement program meets regulatory and CHAP accreditation standards

Requirements

Must-Have:

  • Degree in Chaplaincy, Divinity, Social Work, Counseling, or a related field
  • Prior hospice or palliative care experience
  • Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
  • Minimum 1 year of experience in bereavement, chaplaincy, grief counseling, or end-of-life care

Preferred / Nice-to-Have:

  • Board Certified Chaplain (BCC) or equivalent certification
  • Experience facilitating grief support groups or memorial programming
  • Familiarity with HCHB or similar home health/hospice EMR
  • Ability to serve patients and families of diverse faith backgrounds and traditions
