We're looking for an experienced RN leader to serve as our Hospice & Palliative Care Clinical Supervisor - overseeing clinical operations, supporting field staff, and ensuring the highest standard of comfort-focused care across both service lines. In the interim, this role will also carry a patient caseload, conducting hospice and palliative care visits in the field. This is a meaningful leadership opportunity with a $5,000 sign-on bonus and top-quartile pay.Compensation$41.00 – $45.00 per hour, based on experience$5,000 sign-on bonusFull-time / 40 hours per weekService Area & ScheduleCleveland / Northeast Ohio | Monday – Friday, standard business hours.What You'll DoClinical Supervision:Provide clinical oversight and supervision to Hospice and Palliative Care field staff including RNs, LPNs, aides, and support personnelMonitor caseloads, patient outcomes, and quality indicators across both service linesConduct field visits, joint visits, and competency observations to support staff performance and patient care qualityServe as a clinical resource and subject matter expert for the interdisciplinary teamSupport onboarding, orientation, and ongoing training and development of clinical staffCollaborate with leadership to develop, implement, and refine hospice and palliative care protocols and workflowsEnsure compliance with Medicare, Medicaid, CHAP accreditation standards, and agency policiesParticipate in quality assurance, performance improvement, and regulatory readiness activitiesAssist with on-call coverage and after-hours clinical support as neededField-Based Patient Care (Interim):Conduct hospice and palliative care visits in the field as needed while staffing levels are being built outProvide skilled nursing care in accordance with the established plan of careAdminister medications and treatments focused on comfort, symptom management, and quality of lifeSupport goals-of-care conversations and provide education and emotional support to patients and familiesMaintain accurate and timely documentation in the EMR within required timeframesWhat Sets VNA ApartAI-assisted documentation - spend your time with patients and staff, not paperwork4-star CMS agency with CHAP accreditation and a 124-year legacy in Northeast OhioMileage reimbursement at the IRS standard rate ($0.70/mile)Paid training & CEUs through ReliasLimited weekend/holiday requirements - we respect your time offResponsive support team - scheduling, intake, clinical supervisors, triage, and after-hours coverage so you're never on an islandPublic Service Loan Forgiveness (PSLF) eligible employer - qualifying employees may be eligible for federal student loan forgiveness after 120 paymentsOne of the only home health agencies in Northeast Ohio with a dedicated Behavioral Health nursing programRequirementsMust-Have:Active RN license (Ohio) + current BLS certificationBachelor's degree in Nursing (BSN) required; MSN preferredMinimum 3 years of clinical nursing experience in hospice, palliative care, or a related settingMinimum 1 year of clinical leadership or supervisory experienceReliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service areaFamiliarity with Medicare and Medicaid hospice regulations and CHAP accreditation standardsPreferred / Nice-to-Have:CHPN certification or willingness to pursuePrior experience supervising interdisciplinary hospice or palliative care teamsExperience with quality assurance, performance improvement, or regulatory complianceFamiliarity with HCHB or similar home health/hospice EMRStrong coaching, mentoring, and team development skills
