Nurse Practitioner (NP) - Nursing Home Primary Care (Weekend Coverage)

Location: Brooklyn, NY (Canarsie)
Schedule: Saturday & Sunday
Compensation: $500 - $600 per day (1099)

A medical group providing primary care services to nursing homes is seeking a Nurse Practitioner to provide weekend coverage for a skilled nursing facility located in the Canarsie section of Brooklyn.

This role focuses on providing primary care to geriatric patients in a nursing home environment. The provider will manage patient visits, follow-ups, and coordinate care with facility staff to ensure continuity of care over the weekend.

Schedule

The provider will work weekends (Saturday and Sunday). Providers must be available between 8:00 AM and 6:00 PM, with flexibility in structuring their day based on patient volume and facility needs.

Compensation

  • $500 - $600 per day
  • 1099 independent contractor
  • Additional earning potential depending on patient volume

Clinical Responsibilities

  • Provide primary care services to geriatric nursing home residents
  • Conduct routine and urgent patient visits
  • Manage chronic conditions and acute medical issues
  • Coordinate care with nursing staff and facility leadership
  • Participate in CCM and RPM programs when applicable
  • Document patient encounters in facility EMR systems (SigmaCare or PCC)

Requirements

  • Active New York Nurse Practitioner license
  • Active Medicare and Medicaid provider ID
  • Experience working with geriatric patients preferred
  • Skilled Nursing Facility experience preferred
  • RN background in a nursing home or long-term care setting is highly valued
