Empath Home Health, a member of Empath Health is seeking a compassionate Weekend Baylor RN Registered Nurse - Home Health to work with our Home Health team to deliver individualized care to patients in their homes throughout North Hillsborough & East Pasco counties (North Tampa, Dade City, Land O'Lakes, and surrounding areas).

Home health visits in North Hillsborough & East Pasco counties (including North Tampa, Lutz, New Tampa, Tampa Palms, Wesley Chapel, Land O' Lakes, Dade City, San Antonio, and surrounding areas).

This is a Weekend Baylor role requiring full weekends. You decide which schedule below fits your life!

  • 4 days (Friday-Monday, Thursday-Sunday, OR Saturday-Tuesday)
  • 3 days (Friday-Sunday OR Saturday-Monday)

Perform comprehensive patient assessments, including OASIS assessments at required intervals

Develop, initiate, and revise individualized plans of care as patient needs evolve

Deliver skilled nursing services and treatments as outlined in the plan of care

Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies

Collaborate with the interdisciplinary team to ensure seamless, holistic care

Graduate of an approved nursing program with an active Florida RN license

Prefers BSN or MSN degree

Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred

Prefers experience using Homecare Homebase (HCHB) EMR

Strong assessment, critical thinking, and patient education skills

Willingness to participate in weekend and holiday on-call rotations
