POSITION TYPE: Risk Assessment Provider

SPECIALTY: Nurse Practitioner (NP)

LOCATION: Maine

SHIFT: Flexible Schedule- set your own hours

PAY: $90-$100/ Completed Assessment  


Job Description:

The risk assessment provider is responsible for the documentation and assessments of high risk conditions for patients in their homes. This assessment is necessary for insurance providers to help each member individually with their health and well-being. 


Responsibilities:

  •  Conduct Health Risk Assessments (HRA) on insured plan patients in their homes or plan designated areas
  •  Identify all active and chronic disease conditions as well as determine all physical, mental, and social needs present at the time of the visit 
  •  Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination 
  •  Conduct necessary screenings to capture HEDIS measures and close gaps in care  
  •  Educate and counsel patient and family on any conditions identified during assessment
