Position Overview

This position supports the Director of Nursing (DON) in overseeing the delivery of high-quality care while ensuring regulatory compliance. This role helps manage the department team and promotes a safe, compassionate, and resident-centered environment.

Essential Job Responsibilities:

Resident Care

  • Assist the Director of Nursing in planning, organizing, and directing services within the department.
  • Coordinate services to meet the health needs of the residents while ensuring compliance with federal, state, and local regulations.
  • Perform pre-admission assessments of potential new residents.
  • For at-risk residents, help coordinate the assistance plan and activate resources through home health and communicate with the physician and responsible party regarding issues that keep the resident at risk.
  • Based on physician orders, help guide, evaluate, perform and supervise resident assistance plans including but not limited to, assessments, resident/family teaching, development and updating of residents’ plans of assistance.
  • Monitor resident records and ensure proper documentation according to state and federal guidelines.
  • Complete incident reports and resident documentation, reviewing for completeness and follow up documentation in medical record.
  • Help provide or coordinate educational opportunities related to health issues of interest to the residents and families. (i.e. Alzheimer’s support group)
  • Communicate changes in resident’s overall condition to the responsible party, physician, DON, Executive Director and other staff.
  • With the support of DON, assume on-call responsibilities and address medical emergencies as they arise.
  • Maintain an accurate record of chargeable services and submit record to supervisor by the designated date each month.
  • Monitor safety measures within memory care, including wander prevention and secure environment protocols.
  • Assist residents with activities of daily living, including bathing, dressing, eating, hygiene and mobility.
  • Assist residents with oxygen, ostomies, and other medical problems as outlined in the assistance plan.
  • Assist with medication administer at times, following policy and procedures for medication administration through RN delegation.
  • Document information pertaining to changes in resident’s functional status, personal needs, appointments, etc.
  • Identify supplies needed to perform tasks and report need to Director of Nursing.
  • Support infection control, safety protocols, and emergency preparedness efforts.
  • Other duties as assigned

Staffing

  • Assist with supervision, training, and performance evaluation of nursing department team members.
  • Assist with planning and executing team member schedule.
  • Consult with DON & ED regarding recommendations regarding any disciplinary action and dismissal of team members when necessary.
  • Delegate appropriate nursing tasks while retaining accountability.
  • Help observe and document non-licensed personnel skills as indicated.
  • Review for medication errors and follow up with proper documentation and appropriate disciplinary action.

Operation of Community/Maintaining Census

  • Create and maintain an environment for residents, families and guests that is customer-friendly and that values and respects the uniqueness of each individual.
  • Market the community by developing professional contacts in the local area.
  • Encourage lifestyle activities to promote socialization amongst residents, assisting with the occasional activity
  • Attend regularly scheduled staff meetings and in-services.
  • Report on-the-job injuries to the supervisor before the end of the work shift on the day the injury occurs.
  • Maintain confidentiality of verbal and written information pertaining to residents, community operations and personnel.

Education, Experience, and Other Requirements

  • Active nursing license in the state
  • Minimum of 2+ years nursing experience in senior living, long term care, or assisted living
  • CPR certification

Knowledge, Skills, and Abilities Required

  • Proficient computer skills
  • Strong knowledge of clinical practices, state and federal regulations, and quality standards
  • Ability to manage multiple priorities in a fast-paced environment
  • Excellent assessment, problem-solving, and communication skills
  • Ability to organize and prioritize with good attention to detail
  • Knowledge of HIPAA requirements
  • Proficient in electronic health records systems
  • Ability to travel occasionally for meetings
