Job Description Registered Nurse (RN) Denver, CO Overview: Our well-established home health organization in Denver is seeking a Registered Nurse (RN) to provide comprehensive, skilled nursing care to adult patients in their homes. This role focuses on safe hospital-to-home transitions, individualized care planning, and long-term chronic disease management to improve patient outcomes and reduce rehospitalizations. New graduates and experienced RNs are both encouraged to apply.

Position Offers: Schedule: Monday-Friday, 8:00 AM-5:00 PM, with no on-call requirements Competitive pay: $45-55 per hour (DOE) Malpractice coverage provided Company car provided Paid Time Off (PTO) and Sick Time Off (STO) Key Responsibilities: Perform admissions, discharges, and re-evaluations for home health patients Provide wound care and dressing changes Manage patient medications Conduct thorough patient assessments and develop individualized care plans Support safe transitions from hospital or clinic to home Coordinate with an interdisciplinary team to support long-term management of chronic conditions Maintain consistent follow-through to improve patient outcomes and reduce rehospitalizations Serve as a key clinical resource for patients and families Qualifications: Current Registered Nurse (RN) license in Colorado, in good standing Knowledge of OASIS preferred; training provided Strong clinical assessment and care planning skills Ability to work independently in a home health setting Why Join Us: Bring your clinical expertise to a role where you can truly impact patients' lives in the comfort of their own homes.
