Job description

CareFirstNY is a non-profit organization that provides complete hospice, community counseling, and grief support services to residents of Chemung, Schuyler, and Steuben Counties of New York State.

We’re seeking compassionate and dedicated Physical Therapist to join our Hospice team!

Summary: The Physical Therapist is responsible for providing treatments to patients who are terminally ill, as prescribed by the physician, throughout the CareFirstNY service area: Steuben, Chemung, and Schuyler counties. Therapists will ensure that they communicate, educate, and advocate while performing therapy in a fiscally responsible manner.


Duties:

  • Evaluates, plans, and provides physical therapy treatments for hospice patients.
  • Provides patient/family, RN Case Manager, and LPN verbal and written instructions in physical therapy techniques and equipment, including demonstration.
  • Implements the following services in the home: therapeutic exercise, range of motion instruction, ambulation training, TENS pain control, moist heat, ultrasound, electrical stimulation, JOBST measurements
  • Consults via phone or home visit, with RN Case Manager to evaluate patient progress, if needed.
  • Telephone contact with hospice staff any time physical therapist not able to perform physical therapy component of plan of care or if there is any changes in patient condition.


Skills/Qualifications:

Qualified candidates will possess a current and valid New York State Physical Therapy license, preferably with one year experience. Must have a valid driver’s license and ability to commute throughout the CareFirstNY service area.
