Hospice Registered Nurse (RN) – 7 On/7 Off On-Call


Location: Grand Rapids 

Schedule: Flexible 7 on/7 off on-call shifts (weekdays after business hours and weekends)

Position Overview: 

As a Hospice Registered Nurse (RN), you will provide exceptional end-of-life care to patients and their families, offering both medical expertise and emotional support. You’ll play a key role in an interdisciplinary team, focusing on enhancing the quality of life for patients during this important phase.

Key Responsibilities: 

•Provide compassionate, home-based care to hospice patients with a prognosis of six months or less to live. 

•Collaborate with an interdisciplinary team (physician, social worker, chaplain, CNA, HHA, volunteers) to develop and execute individualized care plans. 

•Conduct thorough patient assessments, monitor vitals, evaluate breathing, and manage pain using prescribed medications and treatments. 

•Ensure the availability of necessary medical equipment, oxygen, and medications. 

•Educate patients and families about care plans, medications, hospice goals, and what to expect. 

•Offer holistic care that addresses the physical, emotional, and spiritual needs of patients and their families. 

Qualifications: 

•Active, unrestricted RN license in the State of Michigan. 

•Minimum of one (1) year of nursing experience (hospice or home health experience preferred). 

•Experience in community health or Medical-Surgical settings (a plus). 

•Reliable transportation, a valid driver’s license, and current car insurance.
