[Stuart, FL] | [Part-Time Saturday and Sunday 8am-5pm]

What You'll Do
  • Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
  • Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.

What You'll Need
  • 1 year of Home Health Scheduling preferred
  • Medical Office Experience a plus
  • Strong communication and organizational skills
  • Ability to work independently
  • Valid driver's license and reliable transportation (if applicable)
  • Commitment to compassionate, patient-centered care
