Continuity of Care Coordinator โ€“ Hospice

VNA Health Group is seeking a Continuity of Care Coordinator to support after-hours hospice operations, including evenings, nights, weekends, and holidays. This role serves as a key partner to the nursing team by coordinating patient care needs, managing communication between care teams, supporting field staff, and ensuring continuity of care across shifts.

Responsibilities
  • Support after-hours hospice field staff and clinical teams
  • Manage non-clinical patient, family, facility, and staff calls
  • Coordinate visits, scheduling, and follow-up activities
  • Ensure timely and accurate documentation
  • Support communication and handoffs between after-hours and daytime teams
  • Escalate urgent issues appropriately
  • Monitor vendor performance and service delivery
  • Assist with onboarding and training of new staff
Qualifications
  • High School Diploma required; Associate's or Bachelor's degree preferred
  • Minimum 2 years of hospice experience required
  • After-hours hospice experience preferred
