What You’ll Do:
  • Perform in-home patient assessments
  • Develop and follow individualized care plans
  • Provide skilled nursing care (wound care, meds, catheter care, chronic disease management)
  • Educate patients and caregivers on home care and safety
  • Monitor patient progress and update care plans
  • Complete timely and accurate documentation

What We’re Looking For:
  • Active Texas RN license (required)
  • Home health or acute care experience preferred
  • Strong clinical judgment and adaptability
  • Excellent communication and patient care skills
  • Reliable transportation & valid driver’s license
  • Kinnser/WellSky experience (preferred)
 Pay & Benefits:
