JOB SUMMARY:

PeopleOne Health is seeking a Registered Nurse (RN) to support patients across the continuum of care through virtual and telephonic nursing services, care coordination, chronic disease management, transitions of care, preventive health initiatives, and patient navigation. The RN serves as a clinical resource to patients and team members while promoting timely access, quality care, and an exceptional patient experience. This strictly remote role focuses on welcoming and engaging new members through their first appointment, ensuring a positive introduction to PeopleOne Health, and supporting timely access to care.

ESSENTIAL JOB FUNCTIONS:

  • Conduct Virtual First Appointments โ€“ Perform new patient intake visits, review health history, establish baseline nursing assessments, and introduce members to the PeopleOne Health model.
  • Utilize nursing judgment to identify patient needs, prioritize concerns, and determine appropriate nursing interventions.
  • Assess reported symptoms and escalate patients requiring provider evaluation or emergency care.
  • Promote Patient-Centered Health โ€“ Educate and counsel patients during virtual visits on self-care, medication use, and follow-up needs.
  • Complete medication reconciliation. Educate patients regarding prescribed medications, side effects, and adherence. Identify medication-related concerns and communicate recommendations to providers.
  • Facilitate referrals and ensure timely completion of recommended services.
  • Support patients with chronic conditions through education, coaching, and ongoing follow-up.
  • Collaborate with Interdisciplinary Team โ€“ Communicate with primary care providers, clinical pharmacists, registered dietitians, health coaches, and care coordinators to ensure seamless handoff and follow-up.
  • Champion Access and Experience โ€“ Serve as a welcoming, accessible first point of contact for patients and contribute to a positive member experience.
  • Perform telephonic and virtual nurse triage using approved clinical protocols.
  • Collaborate with Client Success, Member Experience teams and direct supervisor to maximize impact on patient engagement.
  • Hours of availability to work until 8pm ET two nights per week and weekends to allow for access outside of patientsโ€™ normal work hours.
  • Participate in quality improvement initiatives.

Requirements:

SKILLS & ABILITIES:

  • Strong skills in telehealth-based assessment and triage.
  • Ability to work efficiently in a high-access, episodic care model.

  • Collaborative, flexible, and responsive to patient needs.

EDUCATION & CERTIFICATIONS :

  • Current unrestricted Registered Nurse license in the State of Florida.
  • Graduate of an accredited school of nursing. Bachelor's degree in Nursing (BSN) preferred.
  • Current BLS certification.

EXPERIENCE:

  • Minimum of three (3) years of clinical nursing experience in Primary, Ambulatory, or Telemedicine.
  • Telehealth experience strongly preferred
  • Experience in value-based or team-based care models is a plus
