POSITION SUMMARY

The Campbell Clinic Bone Health Program is seeking a highly motivated and patient-centered Bone Health Nurse Navigator to help build and support one of the region’s most innovative orthopedic bone health initiatives. This role is designed for a nurse who thrives in a collaborative environment and is passionate about preventative care, patient education, and improving long-term musculoskeletal health outcomes.

The Bone Health Nurse Navigator will play a central role in educating, coordinating, and following patients at risk for osteoporosis and fragility fractures. The navigator will work closely with orthopedic surgeons, advanced practice providers, virtual care teams and external referring clinics to ensure patients receive timely evaluation, treatment initiation, longitudinal monitoring, and ongoing support.

This is a unique opportunity to participate in a rapidly growing program focused on the intersection of orthopedic surgery, osteoporosis management, fracture prevention, and emerging screening technology.

Essential Functions Statement(s)
  • Coordinate care for patients identified through fragility fracture pathways, orthopedic referrals, opportunistic imaging and screening initiatives.
  • Perform patient outreach, scheduling coordination, and navigation through bone health evaluation and treatment pathways.
  • Educate patients regarding osteoporosis, fracture prevention, medication options, fall prevention, calcium/vitamin D optimization, and treatment adherence.
  • Assist with pre-operative bone health optimization for arthroplasty and spine surgery patients.
  • Collaborate closely with orthopedic surgeons, APPs, clinic staff, and virtual care partners to streamline workflows and improve patient access.
  • Track patient outcomes, treatment initiation rates, compliance metrics, and follow-up completion.
  • Participate in program development initiatives, workflow optimization, educational meetings, and quality improvement projects.
  • Maintain accurate documentation within the EMR using standardized workflows and templates.

SKILLS & ABILITIES

Education: A graduate of an accredited post-secondary school of nursing is required.

Experience: Prior experience in orthopedics, endocrinology, rheumatology, geriatrics, osteoporosis management, care coordination, or chronic disease navigation is not required but beneficial. Experience with EMR documentation and care coordination workflows preferred

Computer Skills: Must be computer literate. Intermediate computer skills are required.

Other Requirements: Interest in Research would be beneficial. Must be willing to market the program.

Certifications & Licenses: Current TN Registered Nurse License or Multistate Nurse Licensure Compact required. BLS and ACLS are required. PALS preferred.
