Responsibilities

As a Home Health Physical Therapist, You will:ย 

  • Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipmentย 
  • Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or functionย 
  • Maintain communication among the treatment team, physician, patient, and family members as applicableย 
  • Maintain thorough and timely documentation of services provided to ensure proper billingย 
  • Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice actย 
Qualifications
  • Graduate of a Physical Therapist Education program approved by CAPTEย ย 
  • Current Physical Therapist licensure in State and American Heart Association CPR certification required; CPR certification may be offered and completed upon hireย 
  • Two (2) years of experience in adult physical therapy, home health experience preferredย 
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; physical therapy skills per competency checklistย
