Position Overview:
The Registered Nurse at Vital Care serves as a clinical resource for referral sources and community home health agencies regarding infusion therapies provided by our pharmacy. In accordance with state licensure regulations, the nurse may also deliver direct patient care, including assessment, planning, implementation, and evaluation of infusion therapy in collaboration with the registered pharmacist.
This role includes patient education, physician follow up, coordination with pharmacy staff for scheduling and delivery, and may involve advanced clinical procedures such as PICC line insertion (with appropriate qualifications). The position reports to the Nurse Manager.
Key Responsibilities:
  • Administer IV medications and therapies, including subcutaneous injections, following aseptic protocols.
  • Monitor patients for adverse reactions and respond appropriately.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Perform phlebotomy for lab testing.
  • Accurately document assessments, interventions, and outcomes in EHR systems.
  • Educate patients and families on infusion therapies, side effects, and self-care.
  • Participate in quality improvement initiatives to enhance patient safety.
  • Provide support in emergency situations using sound clinical judgment.
Experience & Skills:
  • Experience in outpatient and home care settings across all age groups.
  • Strong understanding of infusion therapy protocols and clinical processes.
  • Background in nursing infusion, emergency medicine, vascular access, or home health.
  • Proficiency in aseptic technique and emergency response.
  • Proficiency with Central Venous Access Devices (CVAD).
  • Ability to perform thorough assessments, administer treatments safely, and maintain accurate documentation.
  • Excellent communication skills for patient education and team collaboration.
  • Critical thinking and adaptability in dynamic healthcare environments.
Qualifications:
Minimum Requirements:
  • Active RN license
  • Graduate of an accredited nursing program (associate or bachelor's degree).
  • Current Basic Life Support (BLS) certification.
  • Clinical experience in a healthcare setting, preferably infusion therapy.
  • Availability to work flexible PRN hours, including evenings and weekends.
Preferred Qualifications:
  • Familiarity with electronic health record (EHR) systems.
  • Knowledge of state and federal healthcare regulations.
