Job description

Life Coach – Permanent Supportive Housing

Location: Bronx, NY

Position Type: Full-Time

Position Overview

Bond Health is currently seeking a compassionate and motivated Life Coach to provide case management and service coordination for adults transitioning to permanent supportive housing. The ideal candidate will have experience supporting individuals with mental health and substance use histories and will help residents achieve long-term housing stability, independence, and community integration.

Key Responsibilities

Maintain a caseload of high-risk residents and provide ongoing case management services.

Conduct assessments, program intakes, and develop individualized, person-centered service plans.

Provide services using Housing First, Motivational Interviewing, Harm Reduction, and trauma-informed approaches.

Assist residents in accessing healthcare, SNAP, SSI/SSDI, public assistance, and other community resources.

Coordinate psychiatric, medical, behavioral health, employment, and educational referrals.

Facilitate life skills, wellness, support, and relapse prevention groups.

Conduct outreach, apartment visits, and resident engagement activities.

Promote housing stability through advocacy, crisis intervention, and collaboration with property management.

Maintain accurate documentation and ensure compliance with program requirements.

Perform other duties as assigned.

Qualifications

Bachelor's degree in Human Services or a related field required.

Experience working with homelessness, mental health, substance use, or trauma-informed services preferred.

Willingness to work one weekend day.

Strong organizational, communication, documentation, and computer skills.

Ability to engage high-need individuals and work independently.

Experience working with low-income or homeless populations preferred.

Bilingual English/Spanish strongly preferred.
