JOB DESCRIPTION:
SUMMARY OF POSITION
Jump start your career in the mental health field as a Psychosocial Educator. Work within a team environment to provide clinical psychoeducation to Centerstone eligible program participants. The responsibility of this adult case manager position is to provide a broad array of services to a case load of SPMI clients who meet eligibility criteria for adult day treatment program. This includes providing group and individual learning activities and support and complete appropriate documentation for all job requirements.
ESSENTIAL DUTIES & RESPONSIBILITIES
  • Manage a caseload of program participants to include completing program assessments, maintaining progress notes, and care plans in the stated timeframes.
  • Attend bi-weekly clinical supervision meetings with clinical supervisor and maintain credentialing status for employment.
  • Ability to facilitate and co-facilitate psychosocial groups and activities with program participants.
  • Documentation to be completed according to standards and agency policies and procedures.
  • Use behavioral health management techniques, crisis intervention, active listening and motivational interviewing to help clients develop better life management skills.

KNOWLEDGE, SKILLS & ABILITIES
  • Effectively communicate via written, verbal, in person and virtual methods.
  • Demonstrate consistently the ability to relate to clients in a culturally appropriate manner.

Bachelor's degree in social work, psychology or relate field required

Experience working with individuals with significant mental illness dx preferred
