Job Summary:

The Licensed Practical Nurse (LPN) Supervisor is responsible for overseeing and coordinating nursing care activities during assigned shifts to ensure safe, high-quality, and compliant resident/patient care. Working under the direction of the Director of Nursing or Registered Nurse, the LPN Supervisor provides leadership, guidance, and supervision to LPNs, Certified Nursing Assistants (CNAs), and other direct care staff while also delivering hands-on nursing care within the scope of the Nurse Practice Act.

Major Duties and Critical Tasks Responsible for: Clinical Care and Oversight
  • Provide direct nursing care in accordance with physician orders, care plans, and facility policies.
  • Administer medications, treatments, and procedures within LPN scope of practice.
  • Monitor resident condition and promptly report changes to appropriate personnel.
  • Assist in implementing and updating individualized care plans.
  • Admission and routine patient/resident observations/transfer notes are complete and accurately reflect the patient's/resident's status.
  • Change in a patient's/resident's physical/psychological condition are reported appropriately.
  • Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
  • Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.
  • Provides patient/resident/family/caregiver education as directed.
  • Initiates emergency support measures (i.e., CPR, protects patients/residents from injury).
Patient Education/Discharge Planning
  • Patient and family teaching is conducted according to the nursing care plan.
  • Explanations are given to the patient prior to intervention.
  • Discharge/death summaries are complete and accurate.
  • Transfer forms are complete and accurate.
Adherence to Facility Procedures
  • Facility Policy and Procedure Manual or reference materials are utilized as needed.
  • Procedures are performed according to method outlined in procedure manual.
  • Safety guidelines established by the facility (i.e., proper needle disposal) are followed.
Documentation
  • The patient's/resident's full name and room number are present on all chart forms. Allergies are noted on chart cover.
  • Unit flow sheets are completed properly (i.e., wound care records, treatment records, weight sheets, etc.).
Medication Administrations/Parenteral Therapy Record
  • Medications are charted correctly with name, dose, route, site, time, and initials of nurse administering.
  • Appropriate notes are written for medications not given and actions taken.
  • The procedure for administration and counting of narcotics is followed.
  • All parenteral fluids, including additives, are charted with time and date started, time infusion completed, site of infusion and signature of nurse.
Coordination of Care
  • Tests are scheduled and preps are completed as indicated.
  • Co-workers are informed of changes in patient conditions or of any other changes occurring on the unit.
  • Information is relayed to other members of the health care team (i.e., physicians, respiratory therapy, physical therapy, social services, etc.) and family/responsible party.
  • Unit activities are coordinated (i.e., changing patients/residents' rooms for admissions, coordinating transfer/discharge forms, etc.).
Leadership & Staff Supervision
  • Supervise LPNs, CNAs, and unlicensed personnel during assigned shifts.
  • Equitable care assignments that are appropriate to patient needs are made prior to the beginning of the shift.
  • Staffing needs are communicated to the nursing supervisor.
  • Assistance, direction, and education are provided to unit personnel and families.
  • Problems are identified, data gathered, solutions suggested, and communication appropriate.
  • Assign duties based on resident acuity and staff competency.
  • Provide coaching, guidance, and support to nursing staff.
  • Monitor staff performance and policy compliance.
Requirements:
  • LPN (Licensed Practical Nurse) licensed by the State NYC
  • Must have 1-2 years of Long Term Care (LTC) required
  • Must have a reliable source of transportation
  • Must have a valid driver's license and automobile insurance
