Job description

Job Description:

The VP, Market Medical Practice (VPMMP) leads and provides clinical oversight to all clinical staff within the applicable market. The VPMMP works in partnership with the Market Vice President (MVP) to manage and execute on market goals including but not limited to, clinical operations, Total Cost of Care (TCOC), care model opex, and service performance goals at the market and partner level for a defined set of contracts.

The Market Dyad (VPMMP + MVP) plays a critical role leading local teams and partnering with centralized functions to deploy, enhance, and maximize the strategic reach of Cityblock's care model. In addition, the VP, Market Medical Practice engages and works closely with senior leaders and executives across the organization in the operational, clinical, and strategic growth/business development spaces.

This role leads with passion for high quality, value-based care, operates and manages a large distributed workforce in a matrixed environment, and leads by example, with empathy, and positive influence to support the continuous improvements of our clinical care services.

Responsibilities:

Strategy

  • Together with the Market Vice President and other internal stakeholders, defines a strategy and market goals that advance contract and company results. More specifically, the VP, Market Medical Practice participates in the planning and execution of the Market Operating Plans.

  • Develops and implements cost of care initiatives based on segmentation and other population health drivers and partners with local care teams to further evaluate and iterate initiatives to support high quality member care, for both consented and pre-consented members.

  • Partners with Cityblock's Data and Analytics teams to understand and map solutions to address drivers of utilization and translate this information to local care teams for iteration of potential data-driven solutions.

  • Leads clinical quality improvement initiatives that may span multiple locations and that focus on HEDIS, contracted or other determined clinical quality metrics, in partnership with Cityblock's Quality Improvement team.

  • Controls total cost of care by implementing the Cityblock Care Model, with a focus on decreasing unnecessary hospitalizations, emergency room visits, and unnecessary speciality referrals.

Clinical Care and Oversight

  • Provides clinical care to an appropriate panel of members with complex needs, with an emphasis on chronic disease management, primary behavioral health, primary palliative care and substance use disorder diagnosis and treatment.

  • Collaborates with Clinical Staff Affairs to manage provider licensing, credentialing and ongoing competency requirements in order to assure market providers are adequately privileged for clinical care.

  • Manages market patient safety efforts and clinical risk management, achieving practice-level and individual patient quality outcomes, in partnership with Cityblock's Member Safety team

  • Participates in the delivery of educational content around complex care delivery including geriatrics, palliative care, chronic disease management, social determinants of care, and integration of primary and behavioral health needs, across the entire care team including non-clinical staff.

  • Meets evolving monthly and quarterly targets for visits with Cityblock members, in both clinic and home settings.

  • Foster clinical mentorship, support and engagement with market providers to minimize voluntary attrition.

  • Ensures clinical cases of providers consistently meet evidence-based standards for complex primary care.

  • Attends case conferences periodically as best fits the clinical/supervisory needs of the market to advise on clinical needs for presented members and establish goals as part of members' care plans.

Partners and network management

  • Attends external meetings and activities to further the business goals of the organization

  • Establishes and maintains network relationships in order to achieve clinical and operational goals through partnership and collaborative planning.

Operations

  • Collaborate with the Market and Hub Operations teams to develop, implement, and monitor clinical workflows and quality initiatives, ensuring clinical needs are met through practice management, while flagging concerns or process issues to promote consistency, reduce variations, and improve efficiency.

Requirements:

Education & License requirements

  • Graduate from an accredited medical college (within the United States/Canada) or from an unaccredited medical college (outside of the United State/Canada)

  • American Board of Medical Specialties certification in a major primary care specialty field of practice

  • Certified in Basic Life Support for Healthcare Providers

  • DEA licensure (or willingness to acquire)

  • Current valid license to practice medicine in the state in which you are employed with Cityblock

  • Willingness to acquire licenses in other states according to CBH business need

Clinical Experience

  • 8+ years experience in direct patient care, demonstrating a strong clinical foundation and knowledge of evidence-based medicine.

  • Experience managing diverse and complex patient populations, including those with chronic conditions, comorbidities, and high-risk physical and behavioral health needs.

  • Specialty Area: MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required

Leadership & Management Experience

  • 4+ years leadership experience in clinical or medical management roles with proven experience leading VBC initiatives, strong familiarity with quality measures (e.g., STAR ratings, HEDIS, NCQA), and in-depth knowledge of risk adjustment models and population health management.

  • Experience leading multidisciplinary teams

  • Experience managing financial metrics and operational processes in a value-based or risk-bearing healthcare setting. Ability to balance clinical quality with financial sustainability.

Preferred Qualifications:

  • Master's Degree: MBA, MPH (Master of Public Health), or MHA (Master of Healthcare Administration)

  • Experience with Telemedicine & Digital Health

  • Experience with addressing Social Determinants of Health
