Guided by our core values of Passion, Respect, and Excellence, the Social Worker supports residents and families by assessing needs, developing care plans, and coordinating services that promote well-being and successful care outcomes.
Responsibilities for Social Worker:
  • Evaluate current, new, and prospective residents to assess psychosocial needs, strengths, and limitations.
  • Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
  • Build and maintain positive, supportive relationships with residents and their families.
  • Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
  • Develop, implement, and update individualized psychosocial care plans for residents.
  • Participate in care planning meetings and support continuity of care throughout the resident's stay.
  • Provide counseling, emotional support, and resource referrals as needed.
  • Ensure compliance with all applicable facility policies, regulations, and documentation standards.

Qualifications for Social Worker: MSW required
  • Bachelor's degree required; MSW degree required
  • Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting required; experience in a nursing home or rehabilitation center strongly preferred.
  • Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
  • Excellent communication and interpersonal skills with the ability to interact tactfully with residents, families, staff, and external agencies.
  • Ability to work effectively in a fast-paced, team-oriented healthcare environment.
