Job Duties
The Community Placement Coordinator oversees and coordinates the timely release of offenders-particularly difficult-to-place cases such as sex offenders, violent offenders, and medically or mentally disabled offenders-who require the development of a home or placement plan when none is available through family or community relationships. The role includes coordinating placement and supportive services for homeless, severely medically and mentally disabled offenders; prioritizing special needs offenders for early placement in assisted living or nursing facilities; assisting institutional staff and probation/parole officers to resolve problematic cases; providing technical assistance on assessments, benefits, and financial resources; participating in VARI re-entry initiatives; and communicating with state, federal, and local agencies to facilitate re-entry planning for disabled offenders returning to the community
Minimum Qualifications
Knowledge of offender re-entry barriers and initiatives. Considerable knowledge of community resources throughout the Commonwealth, public assistance programs (e.g., SSI, SSDI, Medicaid, Medicare, Veterans). Considerable working knowledge of clinical assessment tools (e.g., UAI-Uniform Assessment instrument). Knowledge of state and local criminal justice systems and relevant laws governing institutions, Probation and Parole Districts, and release of offenders. Ability to communicate effectively orally and in writing with a wide variety of agencies, service providers and community members. Proficiency in Microsoft Office to include Outlook, Word, and Excel.
Additional Considerations
Experience working with a variety of organizational units (i.e. Social Services, CSB, and other agencies) that provide supportive services.
